Objective-outcome-KPI matrix

A second-phase artifact linking each expected outcome to the indicator that verifies it.

A second-phase artifact linking each expected outcome to the indicator that verifies it. It makes visible at a glance whether any outcome lacks a means of measurement or any indicator answers to no outcome.

It exposes the most frequent gap: expected outcomes that no indicator verifies. As long as the objective and the measurement live in separate documents, that gap stays invisible until somebody asks whether the process worked.
